2. Fill in and submit the second form to PAY for your order .
NOTE: If you do not fill out and submit BOTH forms, your order cannot be processed!

IF applicable, you may order both Standard size bands and  Jumbo size bands. Please indicate quantities
of each and numbers bands are to start with (READ instructions at left for numbering bands).

band code of up to three
(3) letters, numbers, or a
combination of both
letters and numbers (for
example, 21B, 1B2, A2B).
NUMBERS
If your first order of the
year starts with 01-25
then your second order of
the same year would begin
with number 26. If you
order both standard or
jumbo size bands within
the same order, then
standard size would begin
01-25 and jumbo size
would continue with
numbers 26-51.  Any
numbers can be used each
year, however, they
cannot be duplicated
within that same year. For

NOTE:  Jumbo
size bands are
1/32 larger than
standard size
and are suited
for the larger
exhibition size
cockatiels.   
Bands can take a
week or more to
process so be certain
to order your bands
once you setyour
birds up to breedor
lay eggs.
